Fuel System Oversight Sparks Fire: High-Pressure Diesel Leak Investigation

A picture paints a thousand words:

The vehicle had been at the dealership for some time and the owner had spent about $90K on engine updates and repairs.  On its maiden voyage, the operator could smell diesel and opened the hood and saw fuel seepage from the high-pressure fuel pump to the common rail.  During the repair the service provider had neglected to connect the fuel rail clamp causing the rail to vibrate.

Our examination confirmed this and noted the Ferrel nut had loosened allowing fuel vapors to escape.  We submitted our detailed report referencing our findings, the failure and cause of this fire event.
